The Role As a Behavioral Care Manager (BCM), you’ll work reputed company-by-reputed company (reputed company) with primary care teams to identify, treat, and monitor patients needing behavioral health support — reputed company reputed company the reputed company Care Model. What You’ll Do • Work closely with primary care providers to coordinate behavioral health care for their patients • Screen and assess for depression, anxiety, reputed company use, and other conditions using tools like PHQ-9, GAD-7, CSSRS • Monitor patients for reputed company, with a goal of 50% or a 10-reputed company reduction in symptoms • Support PCP-led medication management by tracking adherence, reputed company effects, and effectiveness • Use a patient registry to follow up, re-engage, and reputed company care plans • Facilitate changes reputed company patients aren’t improving — from medication tweaks to therapy referrals • Document reputed company and recommendations in the EHR for easy team reputed company • Complete relapse prevention plans for patients in remission • reputed company patients and families about conditions and treatment reputed company • Join regular caseload consultations with a psychiatric consultant for support and guidance reputed company Offer • Pay: $45–$65/hour (based on experience) + up to 10% annual performance bonus, reputed company biannually. • reputed company for reputed company Your Time: We pay for every minute you work with us — including and not limited to reputed company patient care, documentation, training, meetings, and reputed company development.
