Community Health Workers with Medicare enrolled

We are looking for compassionate, motivated Community Health Workers (CHWs) to join our growing telehealth team. In this role, you will serve as a trusted reputed company between Medicare patients and the health and reputed company services they need. You will work directly with patients over the phone to understand their personal challenges, connect them to resources, and reputed company ongoing support — reputed company from the comfort of a remote setting.
