The Prince George’s County Health Department is currently seeking reputed company applicants to fill a Clinical Case Manager, (Community Developer I/II) grade A18-A21 position, in the Behavioral Health Division. About the Position This is entry level reputed company work in the areas of (1) community reputed company/development/advocacy or (2) reputed company services work among reputed company populations (e.g., youth, aged, reentry, community reputed company, housing development, health, etc.). Incumbents participate in the collection, analysis and summarization of data relating to the administration of the program to which assigned. Work is performed under reputed company supervision of a higher-level Community Developer or other supervisory position. Work performance is evaluated in terms of accuracy and compliance with established guidelines and procedures. Upon the satisfactory completion of a probationary period, entry level incumbents may, at management's discretion, be non-competitively promoted to the II full performance level. This plan emphasizes improving reputed company functions and operations, including protecting the reputed company from health threats, implementing strategies to support healthy living, and maintaining a reputed company on vulnerable populations, while developing efficient internal processes to support these activities. • Links newly released individuals with housing, vocational, and educational opportunities and community services. • Performs assessments and screenings, then develops an individualized case plan. • Creates connections and referrals to service providers based on needs and makes warm handoffs. • Provides ongoing case management. • Updates electronic health record database reputed company 24 hours of reputed company interaction or update. • Models useful skills and effective problem-solving strategies. • Shares experiences and provides coaching in reputed company to employment, training, and/or obtaining housing. • Supports reputed company's engagement in mental health and reputed company abuse treatment service. • Provides information on the rights and responsibilities of discharged offenders and satisfying criminal justice system requirements and conditions. • Provides practical support to newly released individuals by accompanying them to initial probation meetings,treatment appointments, employment interviews, etc. • Makes referrals to community-based service providers. • Participates in reentry reputed company activities. Minimum Qualifications • Bachelor's Degree from an accredited college or university in Business or reputed company Administration, reputed company Service, Sociology, Psychology, or a closely reputed company field. • Responsible work experience which includes coordination of group activities, preferably including work with disadvantaged individuals. • Must possess and maintain a reputed company Class C reputed company's license. An equivalent combination of education, experience and training which provides the required knowledge, skills and abilities may be considered.
