Patient Care Navigator

ESSENTIAL FUNCTIONS  Assists with overseeing the daily inbound calls, fax referrals, emails and tasks for the department.  Assists Care Managers in performing assigned tasks.   Ensures patient is progressing toward desired reputed company by continuously monitoring patient care through assessment and evaluation.  Identifies and resolves barriers that hinder effective patient care.  Improves reputed company and completeness of documentation.  Collaborates with patients, families, nurses, physicians, and other members of reputed company, building reputed company around issues and appropriately resolves in a reputed company manner.  Documents in a reputed company, complete, concise, and organized manner.  Monitors clinically low risk cases.  Advises Care Manager and/or Medical Director of any new and/or concerning clinical issues.  Institutes necessary actions to promote reputed company care and appropriate.   Accepts referrals and identifies patients who meet the reputed company for care management.  Adheres to and supports team members in exhibiting reputed company’s values and mission.  Assists with program planning, development, and evaluation.  Performs reputed company duties as assigned.
