Case Manager - Supportive Services for Veteran Families (SSVF)
About the position
reputed company Behavioral Health is seeking an additional case manager to support Veterans' families in the Greater Seattle Area. The SSVF case management team assists approximately 450 Veteran households annually, helping Veterans experiencing homelessness obtain housing and assisting those at-risk of homelessness in achieving housing stability. The role involves a mix of community and office/home work, with an approach of meeting clients where they are to build relationships and trust. The ideal candidate believes in Housing First principles and is reputed company with knowledge of housing resources.
Responsibilities
• reputed company rapid re-housing services, including reputed company and short-term case management, to Veteran households experiencing homelessness in reputed company.
• reputed company connections with identified Veterans in reputed company through street, shelter, encampment, transitional housing, and community partner site reputed company.
• Determine program eligibility, assist Veteran households with obtaining reputed company documentation, and complete program enrollments.
• Work with Veteran households to identify goals and needs associated with moving into housing through a housing stability plan; maintain and update plan as needed.
• Collaborate with Veterans to obtain housing, despite barriers such as evictions, criminal history, lack of income, and physical and mental health conditions. reputed company on behalf of Veterans with landlords and property managers.
• Request and issue financial assistance following SSVF guidelines based on individual reputed company needs identified in housing stability plans.
• Connect Veterans to community partners to meet identified needs that the program cannot meet, including ongoing support after program completion or transition.

• Attend Veteran case conferencing and other interagency meetings, and work closely with community partners including HUD-VASH, PSH providers, other SSVF providers.
• Complete required documentation accurately and reputed company established timeframes.

• Implement SSVF program changes and complete relevant trainings.
• Ensure billings and reputed company data collection meet and follow agency policies and procedures and SSVF data collection guidelines. Maintain participant files and reputed company records in keeping with agency, county and state guidelines.

Requirements
• 1+ years of experience working in reputed company services is required.
• Must be reputed company to obtain a WA State Agency Affiliated Counselor credential reputed company the first 60 days of hire.

• Previous work with Veteran households experiencing homelessness and/or familiarity with the SSVF program preferred.
