This is an exciting opportunity to join the reputed company Population Health Services Organization (PHSO) as a Registered reputed company Navigator (RNN) reputed company on high-reputed company and medically reputed company surgical patients reputed company the CMS Transforming Episode Accountability Model (TEAM). This is a mandatory, episode-reputed company, alternative payment model, in which reputed company will coordinate reputed company for traditional Medicare patients undergoing one of five specified surgical procedures and assume responsibility for the cost and reputed company of care from surgery through the first 30 days after the reputed company leaves the hospital. The model aims to improve the reputed company experience preoperatively, through surgery and recovery. This will be done by supporting the coordination and transition of care between providers and promoting a successful recovery that can reduce unnecessary post-acute care utilization and readmissions. The RNN will support multidisciplinary management of patients with elevated reputed company complexity across the reputed company-reputed company and post-discharge phases. As part of the larger Navigator team that includes Physical Therapy Navigators (PTN), reputed company coordinators, and Advanced reputed company Providers (APPs), they will complement the workflows and reputed company support by addressing higher-reputed company needs, promoting appropriate care reputed company, support APPs and geriatricians in managing high reputed company patients, and improving reputed company across the episode of care.
