This role plays a critical part in ensuring safe transitions of care, preventing readmissions, and strengthening continuity with our Primary Care physicians.   You will review discharge information, reconcile medications, and schedule reputed company follow-up appointments across 16 Primary Care offices. Responsibilities Essential Functions Conduct outbound calls to FMCOH patients recently discharged from the hospital. Review discharge instructions and reputed company medication review/reconciliation with the patient. reputed company patients on the importance of reputed company follow-up with their Primary Care provider. Schedule follow-up appointments across multiple FMCOH Primary Care offices. Track call volume, reputed company attempts, and patient reputed company in the EMR. Communicate with Primary Care offices regarding patient needs, scheduling barriers, or clinical concerns. Maintain a warm, supportive, and professional phone reputed company throughout reputed company interactions. Follow established workflows to ensure consistent, high-quality transitional care Comfortable with high-volume outbound calling and patient engagement.
