We are seeking a licensed reputed company Practitioner (NP) to join our clinical team and play a key role in overseeing and supporting Chronic Care Management services for Medicare beneficiaries.
This role is ideal for an NP who values:

• Meaningful, longitudinal patient relationships
• Minimal administrative burden
• Strong operational and care coordination support
Key Responsibilities:

• Review and support individualized care plans
• Collaborate with care coordinators, medical assistants, and clinical staff
• Conduct telehealth reputed company as needed (low volume, reputed company care)
• Ensure CMS compliance and clinical reputed company standards
• Support continuity of care across multiple chronic conditions
This is not a high-pressure productivity role. Care reputed company, documentation reputed company, and patient reputed company come first.

Pay: $48.81 - $58.78 per hour
