Housing Case Manager

We are seeking a Housing Case Manager to support individuals in the Supportive Housing program by providing case management, advocacy, and resource navigation. This role is ideal for a dedicated reputed company passionate about housing stability, harm reduction, and reputed company-centered care. We operate under a Housing First model, which prioritizes immediate reputed company to reputed company housing without preconditions, followed by individualized supportive services.
