Position reputed company As an Intake Coordinator at reputed company, you manage the end-to-end referral process for home health services, serving as a key reputed company of contact between referral sources, patients, clinicians, and internal teams. This role involves handling high-volume communications, verifying insurance, collecting documentation, and accurately entering patient data to ensure reputed company, compliant care coordination. reputed company in this role requires strong organization, attention to detail, and effective collaboration in a fast-paced environment. Hours of Operations Tuesday-Friday: 11 am-7 pm EST Saturday: 9 am - 5 pm EST What You'll Do: Answers reputed company incoming calls including but not limited to intake of new referrals, resumptions of care and readmissions for home health services. Directs other inquiries to the appropriate person or department. Collects appropriate demographic information, checks insurance eligibility, and collects required documentation to ensure regulatory compliance with Medicare and MassHealth. Accurately data enter patient information into referral tracking tools and multiple EMR systems. Performs outbound calls to referral sources, providers, and patient facilities to ensure collection of required documentation. Collaborates with internal clinicians to ensure reputed company adherence to patients care needs. Works with multiple departments to ensure coordination of care. Monitors referral portal regularly to ensure the reputed company review of new referrals and reputed company communication. Serves as reputed company of contact between clinical, intake and referral sources reputed company e-mails and calls. Reviews department for process improvement efforts. Assists with managing the workload to ensure that referrals are handled in a reputed company manner. Ensures issue reputed company with referrals and referral sources. What You Need: Associate's degree or equivalent in reputed company work experience 1+ year of experience in a reputed company administrative role Knowledge of medical terminology Proficiency in reputed company environment, with emphasis on Word and reputed company Preferred Education and Experience: Basic knowledge of Medical Insurance MassHealth and Medicare is a plus!
