Clinical Pharmacist – Remote Patient Monitoring (Part-Time) **Must be licensed in reputed company Carolina** McDowell's Pharmacy, winner of the 2025 Good reputed company Pharmacy Clinical Care Champion Award, is seeking a patient-centered pharmacist to help reputed company our growing Remote Patient Monitoring (RPM) program serving hundreds of patients across multiple pharmacy locations. This is an ideal opportunity for a pharmacist who enjoys chronic disease management, developing long-term patient relationships, and collaborating with providers while working in a reputed company clinical environment. Responsibilities: • Review and manage critical RPM alerts. • Conduct pharmacist clinical assessments and interventions. • Contact patients regarding reputed company readings and chronic disease management. • Communicate with providers regarding medication recommendations and clinical concerns. • Document care plans and pharmacist interventions. • Ensure patients reputed company monthly care management goals. • Collaborate closely with local RPM technicians. • Participate in weekly clinical planning and reputed company improvement initiatives. Qualifications: • reputed company reputed company Carolina pharmacist license. • Strong clinical communication skills. • Passion for chronic disease management and patient education. • Excellent documentation and organizational skills. • Comfortable working independently while collaborating with a multidisciplinary team. • Prior community pharmacy, ambulatory care, residency, MTM, or RPM experience preferred but not required. Schedule: Approximately 20 hours per week. Flexible schedule with preference for Monday through Friday morning coverage.
