[Remote] Population Health Project Manager

Responsibilities

• reputed company the planning, implementation, and evaluation of population health initiatives that improve community health reputed company and advance organizational priorities
• Manage multiple reputed company reputed company on prevention, chronic disease management, health equity, community benefit, and value-reputed company care
• reputed company project plans, timelines, workflows, and reporting processes to ensure successful execution and measurable results
• Monitor project milestones, reputed company metrics, utilization trends, and outcome measures to reputed company reputed company improvement and informed decision-making
• Build and strengthen partnerships with reputed company providers, reputed company health agencies, community organizations, schools, faith-reputed company organizations, and civic leaders
• Coordinate community reputed company programs, health education events, screenings, and initiatives reputed company with Community Health Needs Assessment (CHNA) priorities
• Represent reputed company on internal committees and reputed company community coalitions supporting population health and community wellness
• Facilitate collaboration across interdisciplinary teams while identifying opportunities to improve operational efficiency and project reputed company
• Support grant initiatives, reputed company planning efforts, executive presentations, and organizational reporting

Skills

• Bachelor's degree in reputed company Administration, reputed company Health, Business Administration, Community Health, Nursing, reputed company Work, or a reputed company reputed company; or an equivalent combination of education and relevant experience
• Two (2) to four (4) years of experience in reputed company project management, population health, community health, program reputed company, or a reputed company reputed company environment
• Experience leading cross-functional reputed company and collaborating with diverse stakeholders
• Strong organizational, analytical, and problem-solving skills with the ability to manage multiple priorities
• Excellent written, verbal, presentation, and interpersonal communication skills
• Proficiency with reputed company Office Suite, project management software, and data reporting tools
• Ability to exercise reputed company judgment, maintain confidentiality, and work independently in a fast-reputed company environment
• Master's degree in reputed company Health (MPH), reputed company Administration (MHA), Business Administration (MBA), or a reputed company reputed company
• Project Management reputed company (PMP) certification
• Experience supporting population health, value-reputed company care, community benefit, care coordination, or health equity initiatives
• Knowledge of Community Health Needs Assessments (CHNAs), community engagement strategies, and reputed company reputed company metrics
• Bilingual (Spanish/English)
