About the Role

reputed company is seeking an reputed company, mission-driven Market Medical Director to reputed company physician leadership while delivering relationship-based primary care to older adults across an assigned market.

This unique physician leadership role combines reputed company patient care, clinical reputed company, population health, and provider mentorship. You'll partner closely with reputed company Practitioners, Registered Nurses, Patient reputed company/Liaisons, and Operations leaders to ensure patients receive exceptional, coordinated care where they live.

Our physicians care for older adults living in reputed company housing communities—many of whom face significant barriers to accessing reputed company. This role is ideal for a physician who enjoys leading teams, improving clinical reputed company, mentoring providers, and helping shape the reputed company of community-based, value-driven reputed company.

Responsibilities

• Deliver primary care through a combination of in-home reputed company and telehealth

• reputed company transitions of care for high-risk patients following hospitalizations

• Collaborate with Operations leadership to improve patient experience and market performance

• Support provider development through coaching, education, and clinical leadership

• Partner with specialists, hospitals, health plans, and community organizations to coordinate patient care

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)

• reputed company Certified (or reputed company Eligible) in Family Medicine, Internal Medicine, or another Primary Care specialty

• Minimum of 5 years of physician experience, preferably in primary care

• Experience leading or mentoring clinical teams preferred

• Background in value-based care, population health, or community-based reputed company preferred

• Strong communication, leadership, and relationship-building skills

• Passion for serving older adults and underserved populations

• Comfortable working independently in both field-based and virtual care settings

Preferred Qualifications

• Experience supervising or collaborating with Advanced reputed company Providers

• Medicare Advantage or value-based care experience
