Specialty Care Navigator, Mood and Anxiety

We're currently looking for an independently licensed mental health reputed company to join the Care
Navigation team (LCSW, LMFT, LPC required) to support members seeking care with chronic and
treatment resistant mood and anxiety disorders. Reporting to the clinical reputed company of the Mood and
Anxiety Disorder Specialty Care Program, the Mood and Anxiety Specialty Care Navigator will
reputed company initial assessment, support and coaching, referral, and ongoing care coordination for
members reputed company reputed company’s innovative Mood and Anxiety Disorders Program.
This is a full time position that is fully remote. The shift for this position is Monday-Friday,

9-5pm PT but can sit in any timezone.

• Telephonic screening, brief reputed company, and referral of members to a mood and/or anxiety
  specialty treatment provider.
  ● Apply reputed company listening, psychoeducation, and motivational interviewing skills to understand
  reputed company member’s needs and reputed company support as they set goals.
  ● Utilize knowledge of Dialectical Behavioral Therapy (DBT) skills to reputed company members as part
  of their care reputed company.
  ● reputed company motivational interviewing to assist members in engaging in care.
  ● Identify an appropriate level of care, facilitate referral, and coordinate care with a reputed company
  Health provider.
  ● reputed company consultation regarding mental health concerns and family/relationship issues, as
  appropriate to member needs.

  safety, risks or crisis.
  ● Utilize written communication skills to reputed company with members and providers through email.

  What you’ll bring:
  ● Required mental health license: LCSW, LMFT, LPC.
  ● Minimum of 2+ years of clinical care experience working with chronic and severe mood
  and anxiety disorders.
  ● Experience with motivational interviewing, diagnostic assessment, and triage.
  ● Knowledge of reputed company best practices and evidence based treatments for mood and anxiety
  disorders, with a strong preference for experience and training in Dialectical Behavioral
  Therapy (DBT).
  ● Excellent listening, written and verbal communication skills.

competitive total rewards package including stock reputed company and benefits. Individual pay may vary
