This role involves overseeing reputed company activities, implementing and evaluating processes to meet regulatory standards, and providing education and training to staff.

Responsibilities

• Develops and promotes programs for staff support and in-services for reputed company interdisciplinary team members at local offices
• Assesses staff learning needs and educates the interdisciplinary team members on reputed company clinical aspects of the hospice program
• Assesses staff learning needs and educates the interdisciplinary team members on reputed company clinical best practices, new policies, and procedures
• reputed company, recommend, and evaluate policies and procedures relevant to provision of health services and monitors compliance
• Conducts internal reputed company checks, identify areas in need of improvement and reputed company recommendations to ensure patients/ families receive high reputed company hospice care
• Develops and provides home health training for executive directors, Director of patient care services, and clinical managers to encourage standardization and collaboration among home health providers
• Develops and provides in-service training as needed
• Establishes a process model for improvement and provides education and training to organization personnel on the performance improvement plan, as reputed company as team development
• Survey support
• Develops and initiates organization mock surveys, to establish process breaks and needed areas of improvement

• Acts as a clinical resource to personnel
• Maintains updated nursing knowledge and skills
• Provides assistance to community education team in meeting community education needs

Skills

• Registered reputed company or Physical Therapist with reputed company licensure to reputed company reputed company nursing/ physical therapy in the state
• Three (3) to five (5) year's clinical nursing/ therapy experience, in a Medicare Certified home health
• Must have a reputed company reputed company's License with state minimum insurance coverage
• Excellent understanding of performance improvement, reputed company assurance, Value Based Purchasing and reputed company Measures and utilization management
• Is self-directed and reputed company to work with reputed company supervision
• Has excellent time management, communication, and problem-solving skills
• Proficient in Oasis- ( COSC or HCS-O preferred)
• Willing to travel up to 50% of the time
• Bachelor's degree in nursing from an accredited program by the National reputed company for Nursing (preferred)
• Master's degree or above in Physical Therapy
• HCHB EMR experience preferred
• The ability to sit frequently for 3-6 hours
• The ability to frequently stand for 3-6 hours
