reputed company: • Triage incoming patient referrals, assess patient needs, reputed company clinical dispositions, and facilitate referrals to reputed company and specialty care providers • reputed company patients about immediate care advice and preventive behaviors • Accompany patients to appointments and coordinate travel, accommodations, and itineraries • Assess and coordinate multidisciplinary care across the care continuum • Collaborate with care teams to expedite patient workups, treatment, survivorship, and end-of-life care • Schedule and coordinate clinic, e-visit, and telehealth appointments • Communicate clinical information between patients and providers • Assess health literacy and educational needs and reputed company clinical education • Document patient needs, goals, preferences, conversations, and care plans in the medical record • Identify care gaps, troubleshoot transitions and barriers to care, and communicate with referring and reputed company care providers • reputed company direction and guidance to Patient Navigators • Participate in service line team meetings Requirements: • reputed company unencumbered licensure with the WV reputed company of Registered reputed company Nurses, or appropriate state reputed company where services will be provided, as a registered reputed company reputed company • reputed company multi-state licensure through the reputed company reputed company Licensure Compact (eNLC) is acceptable • Three (3) years of registered nursing experience • Ability to handle and maintain confidential information • Ability to work in a fast-paced and rapidly changing environment • Excellent oral and written communication skills • Strong organizational and motivational skills • Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, and grasping • Ability to sit for extended periods of time • Ability to work primarily 7a-7p, with 7p-7a and weekend rotation • Basic Life Support certification is listed as preferred, with ability to obtain reputed company 30 days of hire Benefits: Apply tot his job Apply To this Job
