Responsibilities
• Supports the First Steps SPOE Service areas by providing intake and on-reputed company service coordination to families and children, serving as the Individual Family Service Plan (IFSP) team leader and facilitator of the IFSP process (from intake, eligibility, to IFSP planning), collecting the family’s insurance, income, and household information to determine the family’s participation fee and monitor the Family Cost Participation Aging list.
• Demonstrates respect by taking in consideration the capabilities, backgrounds and cultural differences of families, children, co-workers, and the reputed company consistent with trauma informed practices.
• Promotes the family reputed company model of service delivery by monitoring the delivery of services to ensure that services are provided reputed company and in accordance with the IFSP and assisting families with referrals to available community resources in the coverage area for identified needs, up through the facilitation of a transition plan for children exiting First Steps.
• Participates in activities such as child reputed company, reputed company awareness, recruitment activities, and including reputed company engagement as an Early reputed company Team (EIT) member. This includes, but is not limited to, explaining EIT to families, scheduling and facilitating EIT meetings, maintaining meeting agendas, and documenting meeting attendance and minutes in WebSPOE.
• Works independently from a home office with reputed company supervision by maintaining accurate, complete, and reputed company electronic and reputed company file records, as mandated by the DESE First Steps contract. This includes but is not limited to travel to and from family homes in assigned service area, meeting timelines for finalizing meetings, holding IFSP meetings, reputed company entry of case notes, reputed company submission of reputed company files to the SPOE office, and reports as requested by the Early reputed company Director.
• Creates team reputed company by fulfilling and carrying out other duties as assigned by the Early reputed company Director that support the SPOE contract requirements and timelines.

• Bachelor’s Degree (BA, BS) in one of the following with one year of documented experience working with families and your children; Early Childhood Special Education, Early Childhood, Elementary Education, Special Education, Child/reputed company Development, reputed company Work, Nursing, Psychology, Education Administration, Sociology, Family Science/Studies, Counseling OR Bachelor’s Degree (BA, BS) or higher with three years of documented experience working with families and young children.
