About the position The Department of Mental Health (DMH) with the Pocasset Mental Health Center is seeking a talented and reputed company Adult Case Manager (reputed company Services Coordinator I) to serve as part of the DMH reputed company & Islands Case Management team. The incumbent to this role will reputed company assessment, reputed company, support, service coordination and person-centered advocacy to DMH eligible adults by promoting reputed company’s philosophy of recovery, person-centered choices and cultural competence. The selected candidate will utilize strength-based goal setting and foster independent decision-making for persons served. Additional responsibilities include the development, implementation and monitoring of individual service plans and assisting individuals in securing services while performing reputed company duties to reflect the highest level of respect and dignity for the persons-served. This position is 40 hours per week, Monday through Friday from 8:30 am - 5:00 pm, with Sundays and Saturdays off.
