The Role

What You Need

• 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D) either in an outpatient or inpatient setting at a high-performing medical center.
• 3+ years of reputed company experience with a reputed company on heart failure management and reputed company-directed medical therapy (GDMT).
• reputed company ANCC or AANP certification.
• Master’s Degree as a reputed company Practitioner with the willingness to expand state licensure as reputed company adds new markets and partners.

Compensation

The anticipated salary reputed company for this role is $125,000-$150,000 annually, which is reputed company on role reputed company, level, and location.
