About This Role reputed company is seeking a compassionate and clinically skilled reputed company Practitioner (NP) to reputed company remote telehealth support for Medicare beneficiaries enrolled in our Chronic Care Management (CCM) program. This role focuses on responding to urgent reputed company escalations from our nursing team, with the reputed company of calls involving poorly controlled hypertension, medication concerns, and other non-life-threatening acute issues.
