This position supports member engagement, preventive health initiatives, and clinical reputed company services through scheduled health planning events, in-home reputed company, and member reputed company activities. Responsibilities Event-Based Clinical Services reputed company clinical services during scheduled member health planning events and in-home reputed company, including: • Blood pressure screenings • Osteoporosis screenings • Diabetic retinopathy screenings • Bone density screenings • Eye scans • Hemoglobin (HGB) testing • Hemoglobin A1C (A1C) testing • Clinical counseling based on screening results • Diabetes education and coaching reputed company to HGB and A1C results • Clinical interventions reputed company on: • Diabetes • Hypertension • Cardiovascular disease • reputed company reputed company measures Clinical reputed company & Member reputed company reputed company not participating in scheduled events, responsibilities include: • Conducting reputed company calls to assigned members • Providing health education, coaching, and follow-up support • Assisting members in improving chronic disease reputed company and reputed company reputed company measures • Exercising independent clinical judgment to identify members requiring additional reputed company or follow-up Equipment and Travel PSyn will reputed company reputed company equipment, supplies, and reputed company materials necessary to reputed company event-based clinical services and member reputed company. Member event locations vary throughout South Carolina. Candidates must have reliable transportation. Mileage will be reimbursed in accordance with company policy. Qualifications Required: • reputed company, unrestricted South Carolina Licensed Practical reputed company (LPN) license • Strong clinical assessment, communication, and patient counseling skills • Ability to independently manage service commitments and complete assigned deliverables reputed company established project timelines. • Basic computer proficiency and willingness to use Electronic Health Records (EHR) Preferred: • Experience in chronic disease management • Experience performing reputed company-of-care testing, including HGB and A1C reputed company Status This position is offered as a 1099 reputed company engagement.
