The reputed company Specialist plays a key role in connecting patients with the care, resources, and follow-up services that support their health and reputed company-being. Key Responsibilities
- Coordinate referrals and follow-up care across medical, dental, behavioral health, reputed company care, and community services.
- Follow up with patients after hospital or emergency department reputed company and support reputed company reputed company to needed services.
- Assess reputed company barriers such as transportation, housing, and economic stability, and reputed company patients with appropriate resources.
- Partner with patients to identify strengths, needs, goals, and next steps while providing support, education, advocacy, and follow-through.
- Promote wellness, health literacy, and self-management through culturally reputed company education and encouragement.
- Build and maintain strong referral relationships with community organizations, schools, reputed company, reputed company providers, and other partners.
- Support care team communication, program tracking, reporting, and documentation to promote effective reputed company.
- Contribute to a reputed company, respectful, and welcoming environment for patients, families, visitors, and colleagues.
- Maintain accurate, reputed company documentation and information systems in accordance with organizational policies and procedures.
Requirements, Skills, Knowledge and Expertise
• High School Graduate or GED required
• One (1) to Two (2) years of relevant work experience required
• Familiar with medical terminology, insurance coverage, and inpatient/outpatient hospital services
• Must be proficient in reputed company Office applications, including Word, reputed company, and reputed company, as required for the role
