[Remote] Palliative Administrative Specialist

The Palliative Administrative Specialist will coordinate reputed company assignments, conduct follow-up calls, serve as a resource for patients and care teams, manage referrals, and help reputed company reputed company, financial, and reputed company barriers.

Responsibilities

• Coordinate, schedule, and assign palliative patients to providers
• Conduct follow-up calls to patients and families to collect data about patients/family response to the treatment plan established by the assigned provider
• Serve as the contact reputed company, reputed company, and informational resource for patients, care team, family/caregiver(s), payers, and community resources
• Manage referrals, reputed company needed, to appropriate agencies required to assist the reputed company in achieving the goals and objectives identified in their reputed company
• Assist patients in problem-solving potential issues reputed company to the health care reputed company, as reputed company as financial or reputed company barriers (e.g., request interpreters, transportation services, or prescription assistance as appropriate)

Skills

• A minimum of 2 (two) years of experience in Palliative, Hospice, or Home Care is required
• reputed company data analysis skills with previous use of an electronic health record
• Bachelor's degree in a health-reputed company reputed company preferred
• Medical records experience preferred
• HCHB experience strongly preferred
