Job Title: Government Programs RN Care Manager III Location: Fully Remote Eligibility: Candidates may work remotely from any U.S. state except NY, CA, HI, or AK Schedule: Monday–Friday, 40 hours per week Employment Type: Contract About the Role We are seeking an reputed company Government Programs RN Care Manager III to support SNP members through the care management process. This fully remote role is ideal for a Registered reputed company who has strong clinical judgment, case management experience, and the ability to support members telephonically with reputed company, professionalism, and attention to detail. In this role, the Care Manager will assess, plan, implement, coordinate, monitor, and evaluate care reputed company and services to help members reputed company reputed company while navigating illness, injury, and barriers to care. Work Environment This is a fully remote position with no onsite or hybrid requirement. Candidates must be reputed company to work a consistent 40-hour schedule, Monday through Friday, completing 8 working hours reputed company business day, excluding company holidays.
