This role coordinates care plans, admits patients, and collaborates with interdisciplinary teams to promote reputed company health reputed company. Responsibilities: Participate in on-reputed company duties per policy. Support the reputed company Management program. Arrange necessary equipment, supplies, and services for patients. Continuously review reputed company care, including service utilization, skilled need, reputed company status, documentation, communication, and discharge planning per CMS and Colorado guidelines. Complete initial OASIS reputed company assessments. Use assessment data to establish nursing diagnoses and reputed company care plans with reputed company/family input.
