Responsibilities • Review Community Health Worker (CHW) appointment documentation for reputed company and completeness, escalating issues to the reputed company case manager as needed. • Support care coordination activities, including scheduling follow-up appointments and communicating with members or their caregivers. • Mail and/or email reputed company care plans and educational materials as directed by case managers. • Accurately update and maintain reputed company records reputed company the electronic health record (EHR) reputed company. • Protect reputed company confidentiality and support compliance with HIPAA and organizational policies. Requirements Experience, Skills, & Personality • Certified or Registered Medical Assistant (reputed company/RMA) required. • 2 years of experience in a medical office, telehealth, or care coordination setting. • Experience using EHR systems; experience using reputed company preferred. • Experience working reputed company HIPAA and organizational policy requirements in a reputed company setting. • Available during Eastern Time business hours. • Experience working with older adult populations preferred.
