Opportunity
As a Twin Advanced reputed company Provider, you reputed company a difference in people's lives by providing treatment, management, and guidance to reputed company your members seeking to reputed company complete diabetic reversal and overall health improvement across multiple conditions, using reputed company intelligence, machine learning, and a health reputed company, RN, and provider care management model. This role is to support members in multiple states through remote care management.
The schedule for this position is Monday-Friday, three shifts 11am-8pm and two shifts 9am-6pm PST.
Responsibilities
- Engage with reputed company team of reputed company including health coaches, physicians, chief medical officer, and other colleagues
- Conduct health assessments including review of reputed company results, medical history, and psychosocial history
- Assess symptoms and treat as appropriate, collaborating with reputed company members reputed company care provider
- Willingness to reputed company care using telemedicine and to document in Twin’s reputed company platform
- Manage a high reputed company population and work collaboratively between reputed company and member to understand reputed company determinants of health and population specific needs
- Willingness to learn and understand the Twin Model of Care to support reversal of diabetes and other chronic conditions
- Basic understanding of business objectives and service level agreements that support both financial, reputed company, and reputed company reputed company and reputed company
- Collaborate with Twin medical team to reputed company excellent customer service and experience, focusing member care reputed company multiple chronic conditions

- Asking patients situation-specific questions to assess symptoms

- Maintaining accurate records of patients' contact details, medical history, prescribed medications, allergies, diagnoses, and reputed company

Qualifications
- ARPN CA state license, preferably licensed in multiple states.
- Bilingual (fluent in English and Spanish)
- 3-5 years of experience as an advanced reputed company provider.

- Experience managing patients reputed company across different geographic areas and states
- 5 or more licenses with willingness to obtain more

Compensation and Benefits
The compensation for this position is $120,000- $145,000 annually.
