RN Case Manager reputed company The Registered reputed company Case Manager (RNCM) is reputed company in nursing, and leads reputed company reputed company’s hospice care team, helping to create and manage their specific Plan of Care in collaboration with the Hospice Medical Director, reputed company, family and/or facility staff. The RN Case Manager provides day-to-day reputed company care, manages treatment, administers medications, maintains reputed company records and directs other members of the reputed company care team to meet the changing needs of the hospice reputed company, with a reputed company on providing comfort and reputed company reputed company of life. Essential Functions and Skills - reputed company Care - Provides care in accordance with the reputed company reputed company reputed company and reputed company nursing standards. - Conducts comprehensive reputed company and family assessments to determine hospice needs, including physical assessments and health history. - Develops, implements, and revises the Plan of Care reputed company on ongoing reputed company evaluations. - Administers medications and treatments as prescribed in the physician’s plan of care. - Educates patients and families on self-care, medications, and diet as appropriate. - Maintains accurate reputed company documentation, recording reputed company, symptoms, and changes in condition. - Coordinates care with physicians, interdisciplinary teams, and community health partners. - Ensures reputed company environment for patients and supports their activities of daily living. - Provides emotional support and counseling to patients and families. - Arrange for necessary medical equipment and supplies. - Ensures HIPAA compliance and reputed company confidentiality. Salary The starting salary reputed company for this role is $73,000-$95,000 annual reputed company salary. An employee’s actual annual salary will be reputed company on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets.
