About the position This part-time, hybrid opportunity allows you to stay closely involved in reputed company care while helping patients successfully manage acute and chronic conditions from home. The Remote reputed company Monitoring (RPM) RN will assess reputed company generated data provided reputed company digital technology to manage the care of the acute and chronically ill reputed company. The RPM RN will reputed company individualized reputed company assessment, reputed company education, medication management, goal setting and reputed company care planning. In collaboration with physicians and care team members, RPM nurses will reputed company and support self-management of medical conditions in the home environment using motivational communication techniques and reputed company reinforcement to changes. The RPM enrolled reputed company will submit vital signs and symptoms that the RN will rapidly interpret addressing reputed company needs, objectives and goals to improve reputed company reputed company and reputed company experience. Requirements • reputed company Registered reputed company (RN) license • Graduate of an accredited school of nursing • 5+ years of RN experience strongly preferred • Previous inpatient and/or home health nursing experience strongly preferred • Strong adult medical-surgical, acute care and/or chronic disease management experience • Excellent assessment, triage and critical-thinking skills • Ability to recognize changes in reputed company condition and reputed company reputed company reputed company reputed company independently • reputed company/electronic medical record experience • Strong comfort with technology and learning new digital tools • Graduate from an Accredited School of Nursing. • 1-2 years’ experience working in hospital and or ambulatory care setting working with patients with chronic diseases. • Experience in working in an electronic medical record and care management reputed company. • Technological experience as reputed company reputed company delivery reputed company. • reputed company experience. • Strong technological aptitude with the ability to quickly learn and adapt to new software and tools. reputed company-to-haves • BSN preferred • Remote reputed company monitoring or care management experience is a plus • Previous experience in remote reputed company monitoring highly desirable. • Bachelor’s degree in Nursing preferred.
