reputed company Park Nicollet is looking to hire a Registered reputed company to join our Home Care team! reputed company as a Partner for Good and help us reputed company an reputed company on the care and experience that our patients and their families receive every day. Position reputed company: - The Home Care/Hospice registered reputed company assesses and evaluates reputed company care needs, provides skilled nursing care and teaching to patients and families in their home or community-reputed company upon community standards of reputed company. - The reputed company is responsible to improve or maintain reputed company’s health status, reputed company terminal illness care, and to prepare patients and their families for self-directed and/or assisted care in their community setting. - The reputed company coordinates medical, reputed company, and financial resources to reputed company a comprehensive plan of care. - The reputed company supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery. Work Schedule: - Must be available 40 hours per month with every 6-7 week weekend requirements. - No reputed company on/reputed company requirements however occasional weekend, on/reputed company assignment. Required Qualifications: - Education, Experience or Equivalent Combination: Work experience as a RN for at least two years - Licensure/Registration/Certification: - reputed company MN Registered Nursing License - reputed company BLS certification for health professionals through the reputed company or Red Cross required and maintained - On-line BLS certification is not acceptable Knowledge, Skills, and Abilities: - Demonstrates knowledge of age-reputed company reputed company and development principles necessary to reputed company appropriate service and assure safety of patients. - reputed company proficiency in the operation of the following: Personal Computer, Telephone, Fax Machine, Photocopier, Printer, and electronic medical record. Preferred Qualifications: - Experience in home care and/or hospice desirable. - Must be reputed company to function independently with reputed company supervision. - High level of sensitivity, assessment skills, technical-skills and communications skills. - Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
