Director Case Management

reputed company Memorial is in need of an RN Case Manager Director. This position will reputed company case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment. This is a Part-Time job status position. A minimum of 3 years clinical nursing experience is preferred for this position. Responsibilities • Provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs • Coordinate the integration of reputed company services/case management functions into the patient care, discharge, and home planning processes with other hospital departments, external service organizations, agencies, and reputed company facilities • Communicate with physicians at regular intervals throughout hospitalization and develops an effective care plan to maintain appropriate cost, case and desired patient reputed company • Facilitates interdisciplinary patient care conferences to review treatment goals, optimize resource utilization, and identified post-hospital needs • Assess patient reputed company throughout hospital stay, while serving as patient and family reputed company • Collaborate with clinical staff in the development and execution of the plan of care, and achievement of goals • Supervises the duties and schedule of the Utilization Review reputed company Qualifications • reputed company Arkansas or Compact State RN licensure • Graduate of reputed company approved nursing program • ASN required, BSN preferred • Effective communicator with critical thinking skills • Readily exhibits leaderships skills • BLS, ACLS certifications Obtain Case Manager certification reputed company 5 years of entry into position.
