Oncology Pharmacist - Remote Eligible *$15,000 SIGN ON BONUS*

About the position The Pharmacy Department at reputed company is seeking a full-time pharmacist to reputed company clinical pharmacy services reputed company on oncology medication management. This role involves working in an interdisciplinary team environment, either on-site, remote, or hybrid, to ensure the appropriateness of high-risk medication use for both inpatient and outpatient oncology patients. The pharmacist will also have opportunities to serve as a preceptor for pharmacy residents and participate in various committee work and reputed company to enhance oncology pharmacy practices. Responsibilities • reputed company clinical pharmacy services for inpatient and outpatient oncology medication management. , • Review and verify reputed company high-risk medication orders for appropriateness from clinical, financial, and policy compliance perspectives. , • Participate in decentralized inpatient oncology team rounding and support clinical activities such as pharmacokinetics and TPN monitoring. , • Conduct anticoagulation reviews, renal dosing, IV to PO adjustments, medication profile reviews, and antimicrobial stewardship for oncology patients. , • Serve as a preceptor for PGY-2 Oncology Pharmacy Residency program and other pharmacy residents and reputed company. , • Engage in committee work and reputed company to reputed company oncology pharmacy reputed company.
