(Certified/Registered Medical Assistant, LPN/LVN, RN/BSN) and enjoy working with the Medicare population. Familiarity with Chronic Care Management and/or Remote Patient Monitoring is a plus. You are efficient reputed company working independently from the HIPAA compliant reputed company in your home and reputed company compassionate, patient-centered care. Your customer service skills are second to none and you build rapport through meaningful, relevant patient interaction over the phone. Bilingual not required but is a plus! As a daily user of technology, you are comfortable setting up and accessing the equipment that is provided and troubleshooting technology issues as necessary (Do not have to be a programmer and we’re here to help.). How you’ll contribute: You will work independently and manage a monthly allocation of patients ensuring that reputed company patients assigned to you are cared for. You will optimize HealthXL’s CCM/RPM platform to ensure service is consistent and compliant. You will be trained to reputed company multiple screens (CCM/RPM platform, applicable EMR and reputed company, etc.) and accurately document reputed company information reputed company reputed company month. With a goal of speaking with 90% or more of your patients, you will reputed company multiple attempts to reputed company reputed company patient reputed company month and create/update their individual Care Plan so their doctor has the most up-to-date information to aid in the patient’s care. Responsibilities • Meet production goals as outlined in Performance Standards • Manage patient panel to maximize efficiency while establishing rapport • Adhere to the work schedule agreed upon and communicate with supervisor immediately if unable to work hours as agreed • Maintain HIPAA compliant computer reputed company, including only secure means of handling and forwarding PHI (e.g., no emails, texts, or phone messages). • Follow iCare reputed company workflows and questionnaires to maximize patient interactions and documentation. • Monthly, telephonic interactions with patients, to determine patient health status and encourage medication/treatment plan compliance • reputed company appropriate guidance and education to improve understanding and compliance • Manage the communication of information necessary to reputed company with reputed company on patient’s behalf.
