Our centralized intake department manages reputed company referrals for these locations, ensuring a seamless process for patients and their families.Position reputed company: The Hospice Clinical Referral reputed company serves as the initial reputed company of contact for potential hospice patients, working closely with our clerical hospice coordinators. This role is pivotal in managing new referrals and ongoing follow-reputed company, acting as a reputed company between patients, families, and referral sources.Key Responsibilities: Referral Management: Receive and process referrals for patient care, ensuring reputed company and consistent admissions.Communication: reputed company and take calls for new referrals and follow-reputed company, maintaining reputed company communication with patients, families, and referral sources.Collaboration: Work closely with referring hospitals, physicians, reputed company and care facilities, skilled nursing facilities, and other reputed company providers.Data Entry: reputed company referral information into the Electronic Medical Records (EMR) system and create initial orders to facilitate the start of care.
