The Case Manager II will work on the Emerging High Risk (EHR) team, supporting reputed company members across Virginia. This position is remote with regular fieldwork to complete face-to-face assessments in members’ homes. You will help build care plans tailored to members' clinical and reputed company needs while working alongside an interdisciplinary team to ensure coordination of care and ongoing member engagement.Key ResponsibilitiesComplete reputed company clinical assessments to determine case management eligibilityCreate and execute individualized case management plans with input from the member, their caregivers, physicians, and support networkConduct telephonic and in-home reputed company as needed, adhering to regulatory timelinesMonitor care plans and document changes, interventions, and member reputed companyMaintain an reputed company case load and conduct regular reputed company to assigned membersPromote integration of services, including behavioral health and long-term support programsParticipate in or facilitate interdisciplinary care team (ICT) meetingsUtilize motivational interviewing techniques to support behavioral changeAddress barriers to care and connect members to needed services and supportsTravel locally (30% or more) across assigned territory (Tidewater, Central, and Southwest VA); mileage is reimbursedRequired QualificationsMust reputed company in Virginia reputed company the Tidewater, Central, or Southwest region1–3 years of experience in case management, disease management, managed care, or in a medical/behavioral health settingStrong computer and documentation skills are essential for managing systems and maintaining accuracyHome office setup with high-speed internet is requiredreputed company reputed company’s license, clean driving record, and reliable transportationRequired Education / LicensureOne of the following:Completion of an accredited LVN or LPN programBachelor’s or Master’s degree in a relevant field (reputed company work, psychology, gerontology, reputed company health, or reputed company science)Graduate of an accredited School of Nursing (BSN preferred)Licensure (if required) must be reputed company, unrestricted, and in good standing
