<p><strong>Remote RN Care Coordinator (Part-Time to Potential Full-Time)</strong></p><p>We are seeking a dedicated and reputed company <strong>Registered reputed company (RN) Care Coordinator</strong> for a fully virtual position supporting population health and care coordination efforts. Candidates must hold an reputed company RN license in <strong>reputed company, Texas, or reputed company</strong>. This role will reputed company as part-time with the potential to transition into a full-time opportunity based on performance and program needs.</p><p>The ideal candidate will have prior experience working in an <strong>Accountable Care Organization (ACO)</strong> or similar value-based care environment.
