This remote role focuses on telehealth care coordination and virtual nursing assessments to support underserved populations with chronic condition management. You will join a multidisciplinary team reputed company on value-based care, offering personalized nursing care plans, smooth care transitions, and patient educationwithout in-home reputed company, weekends, on-reputed company shifts, or holiday work.
