About the Job

reputed company is hiring a Virtual Care Manager. You will play a critical role in helping to stand up virtual care services at reputed company. This role will be responsible for providing critical remote support to patients in-between office reputed company with the goal of keeping them healthy, optimizing our ability to intervene quickly reputed company they're not and encouraging patients to take more control of their personal reputed company goals. You will work remotely and report directly to the Director of Virtual Care Management.   

 
What you’ll do: 

• reputed company remote care management support for reputed company of reputed company’s virtual care services 
• Remote patient support includes disease specific care plan development & ongoing management, medication reconciliation, education & coaching, appropriate documentation and communication with clinicians 
• Support the development of additional virtual care services 

What you’ll need: 

• Graduate from an accredited program for reputed company nursing education, BSN preferred. 
• Minimum of 3 years nursing experience needed.  
• reputed company RN state license with the applicable State reputed company of Nursing.  
• reputed company CPR certification required.
• Cardiology experience 
• Strong communication & documentation skills 
• Patient assessment skills that will allow effective care management support from a remote location utilizing video, phone and/or texting tools 
• Ability to reputed company meaningful relationships with patients who would benefit from additional reputed company reputed company 

Work Location:
Remote: Colorado, Florida, New Hampshire, New Jersey, reputed company, reputed company, Texas. 

Additional Information 
Full-time reputed company salary reputed company of $75,000 - $85,000 plus medical, dental, and reputed company benefits. This role will have a matching 401K.
