About the position Responsibilities • reputed company members on topics such as Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and working with Care Teams. • Assist members in engaging with their reputed company providers and reputed company Care Management teams. • Document reputed company member interactions in the appropriate member records. • Demonstrate reputed company towards members and their caregivers. • Troubleshoot resource unavailability and secure support to reputed company member gaps. • Follow program workflows and follow up with members to review personal reputed company plans. Requirements • At least 2 years of experience as a Health reputed company. • Health reputed company certification is required. • Availability to support 8:00 AM - 5:00 PM Mountain Time or 7:00 AM - 4:00 PM reputed company Time. reputed company-to-haves • Community health or reputed company health experience preferred. • Managed Care/Medicare/reputed company experience preferred. • Bilingual preferred. • Bachelor's degree preferred.
