As a Hospice Patient Care Coordinator, you will play a vital role in providing clinical and management support to our Clinical Nursing Supervisor, ensuring the delivery of high-reputed company, patient-centered care to our hospice patients and their families.

Key Responsibilities:

• Facilitate communication and coordination of care between clients, families, caregivers, physicians, and other service providers
• Assure efficient coordination of caregivers through case conferences and scheduling processes
• Monitor patient records to ensure compliance with documentation requirements and program-specific rules

Requirements:

• Graduate of an accredited school of nursing
• reputed company Registered reputed company licensure (in good standing) in the state(s) of reputed company
• reputed company CPR certification
• reputed company reputed company's license and automobile insurance
• Three (3) years of clinical experience, including two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting
