Remote Care Navigator (LPN/LVN) - EST Time Zone - Michigan and Compact License

In this exciting role, you will utilize your clinical expertise and motivational communication skills to build trust with patients, promote lifestyle changes, and develop personalized action plans. Collaborating with care teams, you will conduct wellness calls, educate patients on disease management, and provide preventive health coaching. Your expertise will focus on triaging vital signs, managing physiological markers, and leveraging remote patient monitoring (RPM) and Chronic Care Management (CCM) principles to drive patient engagement and reimbursement requirements.
As a Care Navigator, you will:
- Manage physiological markers, such as blood pressure, weight, and blood glucose, with clinical appropriateness
- Meet team goals, standards, and metrics, significantly impacting longitudinal patient engagement in RPM programs
- Provide preventive health and disease management education, coaching, and monthly wellness calls to assigned patients
- Lead collaborative wellness calls, defining health goals outlined by care teams, and manage patient messaging and alerts
- Direct patients to treating physicians for routine questions and follow escalation pathways for urgent care needs
To succeed in this role, you should have:
- An unrestricted LPN/LVN license in a compact state and Michigan
- Ability to work in the EST time zone (8:00 a.m. - 5:00 p.m.)
- 1-3 years of clinical experience in patient management, disease management, or a related field
- Experience working with different provider practices, workflows, and Medicare patients
- Strong knowledge of RPM, CCM, and telephonic care management principles
- Excellent communication, motivational interviewing, and health coaching skills
