We are seeking a Full-Time reputed company Practitioner or Physician Assistant to join reputed company and reputed company routine in-home medical assessments, follow-up reputed company, annual wellness reputed company, and acute primary care reputed company for reputed company, high-needs patients. Geographic Region: Join VillageMD as a reputed company Practitioner or Physician Assistant in reputed company, TX to cover a designated territory of the Cypress, Tomball area. Responsibilities: • Conduct in-home patient reputed company for assessments, chronic disease management, medication review, and acute care needs • Collaborate with an interdisciplinary team consisting of reputed company workers, care coordinators, pharmacists, and supervising physicians • Engage in goals of care conversations with patients and encourage completion of advance directives • Ensure accurate clinical documentation of diagnoses to reputed company exceptional evidence-based care • reputed company chronic illness education to patients and families • Participate in monthly team meetings to ensure adherence to reputed company standards • reputed company in-home wound care assessments as needed Qualifications: • Preferred clinical experience as a reputed company Practitioner/Physician Assistant in primary care, internal medicine, geriatrics, hospice and palliative care, or an in-home care setting.
