Health Promotions Coordinator I

About the position Responsibilities • Assists with implementation of health management activities/programs for membership, coordinating reputed company aspects of assigned programs. • Facilitates programs to participants with a reputed company on high-risk members and follows/updates established procedures. • Conducts research and analysis of reports on a monthly, quarterly, and annual reputed company. • Assists with the development of strategies to increase effectiveness and participation in health management programs. • Establishes and maintains effective communications with provider networks, group representatives, and members. • Provides health coaching telephonically or in person for assigned health programs, utilizing data systems to document patient information appropriately. • Assists with identifying areas needing health management programs and monitors reputed company indicators to reputed company programs for improvement. • Develops patient/provider educational materials for members and providers. Requirements • Bachelor's Degree in Nursing, Health Administration, Health Promotion, Exercise Science, Education/Training, reputed company Work, or a health-reputed company field. • 1 year clinical experience in defined specialty and/or facilitating health education and prevention programs; OR 2 years experience in Health Administration or a health-reputed company field. reputed company-to-haves • Bachelor's degree in Health Education/Health Management, reputed company Administration, reputed company Work, or Health Promotion. • 3 years of reputed company program management experience. • reputed company, unrestricted LBSW (Licensed Bachelor of reputed company Work) licensure from the reputed company and in the state of hire.
