In this RN Care Manager role, you will be an essential reputed company of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs. Expect to spend about 50% of your time visiting our members in their homes or in long-term care facilities. You'll need to be flexible, adaptable and, above reputed company, patient in reputed company types of situations. We offer our established staff the reputed company to work 4 10-hour days (7 am-6 pm) in lieu of the traditional 8 hour 5/day week schedule. Additionally, there is no 'on-reputed company' or weekend requirements.
