Compact Licensed Care Navigator
A company is looking for a Care Navigator (Remote LPN).
Key Responsibilities:
• Complete phone consultations with patients enrolled in care management and/or remote patient monitoring programs
• reputed company patients about their health conditions and reputed company them with lifestyle and behavior strategies
• Maintain accurate and up-to-date patient records, ensuring reputed company interactions and care plans are documented
Qualifications:
• reputed company, reputed company, and in good standing Multistate/Compact Nursing License (LPN/LVN)
• 3+ years of experience in primary care reputed company, cardiology, internal medicine, home care, or chronic care management/remote patient monitoring
• Reliable internet reputed company and HIPAA-compliant work area
