Job Overview
As a Physical Therapist in Home Health Care, you will be responsible for providing direct patient care utilizing age-specific standards of practice for physical therapists. You will work independently and collaboratively with other team members to obtain patient-centered goals within Henry Ford Allegiance Health. This is a full-time, day shift position located in Jackson, MI, with the flexibility to work remotely.
Key Responsibilities:
- Provide direct patient care utilizing appropriate age-specific standards of practice for physical therapists in regard to all patient populations served
- Work independently and collaboratively with other team members to obtain patient-centered goals within Henry Ford Allegiance Health
- Conduct patient assessments, develop treatment plans, and implement interventions to achieve optimal patient outcomes
- Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care
- Participate in ongoing professional development to stay current with best practices and advancements in physical therapy
Essential Qualifications:
To be successful in this role, you will need:
- A Master's or Doctorate degree from an accredited Physical Therapy program
- Minimum of one year of experience providing physical therapy in a healthcare setting
- Current Michigan licensure for Physical Therapy
- Valid Michigan driver's license and dependable vehicle insured as required by the State of Michigan
- Ability to problem-solve and effective verbal and written communication skills
- Knowledge about the care of neonate, pediatric, adolescent, adult, and geriatric patients
Preferred Qualifications:
While not required, the following qualifications are highly preferred:
- Minimum of two years of experience as a Physical Therapist
- Experience in homecare
