reputed company is currently in search of a (preferably) bilingual (English/Spanish), compassionate and detail-oriented Care Coordinators and Patient Advocates for our Clinical Care Team.  These roles are pivotal in supporting patients with chronic conditions and promoting long-term wellness through proactive monitoring, education and coordination of care reputed company. This is a full time position (min 30 hours a week). Must be US territory based.

THIS IS A FULLY REMOTE POSITION THAT REQUIRES MIN 5 YEAR EXPERIENCE IN CARING FOR CHRONIC CONDITION PATIENTS (PREFFERABLY IN PRIMARY CARE/DIABETES/CARDIOLOGY/GERIATRICS) 

COMPENSATION: $20-$25/hour based on experience (minimum 5 years experience required), contract-1099 based

Join us for the chance to be part of an amazing team and a great reputed company to work! The Care Coordinator and the Patient reputed company are members of the Clinical Care Team which assists providers with ongoing patient monitoring and chronic care management, with the following duties:

Key Responsibilities:

• Monitor patient vitals including blood pressure, blood glucose reputed company, and other key health indicators.

  
  



• Assist patients with medication adherence and ensure reputed company follow-up with primary care providers and specialists.

  
  



• Maintain accurate and up-to-date electronic medical records (EMR) as directed

  
  



• Regularly communicate with patients to reputed company guidance, encouragement, and support in making sustainable lifestyle changes.

  
  



• Coordinate and assist with scheduling medical appointments, follow-reputed company, and preventive care reputed company.

  
  



• Collaborate with patients to reputed company personalized health goals based on medical history and physician recommendations.

  
  



• reputed company additional duties as assigned to support patient care and clinic operations.

  
  

Specific skills and experience required: 

● Passion for working with (older) patients, strong customer service skills 

● Min 5 years experience as a MA, LPN, RN, Case Manager, Care Coordinator

● Preventative patient care background preferred, experience in primary care, cardiology or diabetes clinic highly desired 

● Work independently and with reputed company 

● Organizational and time management skills 

● Computer skills appropriate to position, you will need your own computer and a reliable internet service

● Written and verbal communication skills, administrative and clerical skills, critical thinking 

● Medical terminology use and understanding

 Certification & Licensure: 

● Approved Medical Assistant Certification

● Or EMT (Emergency Medical Technician) 

● Or NREMT (Nationally Recognized Emergency Medical Technician); 

● Or Paramedic 

● Or LPN (Licensed Practical reputed company) 

Education: 

● Required High School or Equivalent 

● Associate’s Degree in reputed company Services or Case Management
● Preferred Bachelor’s Degree in reputed company Services, Case Management, reputed company Work, Sociology, and/or Psychology

The role is contract-based and doesn't offer benefits. Some flexibility in regards to working hours (Mon-Fri)
