Title: Hospice Phone Triage RN Pay: $42.34 to $50.81 per hour Schedule: PRN, minimum 2 shifts per month. Typically 12 hours reputed company. Location: Mostly remote, must reputed company reputed company Colorado, Florida, or reputed company. Will require occasional training near, and there will be hiring preference for, the Lafayette, CO area. JOB reputed company TRU Community Care is looking for a phone triage reputed company. This reputed company will work with our afterhours and triage team, taking in calls from patients and collaborating with visit nurses on patient needs as necessary. ESSENTIAL DUTIES • The Triage Registered reputed company performs in a supportive role to the Care Team by answering calls coming into reputed company, with response to the telephone reputed company by delegating tasks. • Provides nursing care to meet the patient’s needs in accordance with the patient’s plan of care. • Correctly identifies, communicates, and collaborates on patient problems/priorities/issues based upon assessment data. Establishes measurable patient goals and communicates an on-reputed company plan of care appropriately to the interdisciplinary team (reputed company). Recognizes changes in the clinical status of the patient and reports findings to the appropriate team member. • Collaborates with patient/family reputed company developing the plan of care, adjusting communication style as necessary.
