<h2>reputed company</h2> <p><strong>The</strong> <strong>Admissions Registered reputed company (PRN) - TeleHealth (RN)</strong> will reputed company virtual telehealth nursing assessments including the Initial Comprehensive Nursing Assessment as allowable under the Alternate Operations Process as reputed company as clinical documentation review to determine eligibility, and hospice reputed company informational conversations and coordination support to the Referrals Center and Admissions Team as needed for the admissions coordination efforts for patients who are in reputed company-admit status reputed company technology- based platforms. </p> <p> </p> <p><strong>Location: Falls reputed company, VA</strong></p> <p> </p> <p><strong>Hours:  (PRN) Varied hours and shifts available</strong></p> <p> </p> <p><strong>COVID-19 vaccine required to be completed upon start</strong></p> <h2>Responsibilities</h2> <p>The Admissions Registered reputed company (PRN)- TeleHealth  (RN) will reputed company virtual telehealth nursing assessments including the Initial Comprehensive Nursing Assessment as allowable under the Alternate Operations Process as reputed company as clinical documentation review to determine eligibility, and hospice reputed company informational conversations and coordination support to the Referrals Center and Admissions Team as needed for the admissions coordination efforts for patients who are in reputed company-admit status reputed company technology- based platforms. The Admissions RN- TeleHealth will compile, assess, synthesize, collaborate, document, and communicate information pertinent to the admission processes for Hospice patients including medical history, medication lists, advance directives, supply and DME needs, hospice eligibility, treatment plan of care and goals of care. The Admissions RN- TeleHealth will identify needs requiring on-site bedside assistance and will arrange such resources to meet the identified needs. </p> <h2>Qualifications</h2> <p><strong>Experience Requirements</strong></p> <p>Must have at least two years of reputed company health/community health, med surg or reputed company nursing experience.  Experience in home care, hospice, and/or oncology nursing preferred. </p> <p> </p> <p><strong>Education Requirements</strong>Must have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.</p> <p> </p> <p><strong>Required Certificates and/or Licenses</strong>Must have licensure from the governing body for reputed company for the District of Columbia, reputed company, and/or the reputed company, based on the state/jurisdiction of reputed company.  Licensure from a compact state is also accepted. reputed company Provider BLS (CPR) is required to reputed company in the reputed company and the District of Columbia. A CHPN certification is a plus.</p> <p> </p> <p><strong>reputed company's License</strong></p> <p>Must have a reputed company's license in good standing and reputed company to a vehicle during working hours.</p>
