Remote Telehealth Compact and State Specific - PM- EO

A leading reputed company reputed company is seeking Remote Registered Nurses to support a new program that reviews health risk assessments and supports HEDIS gap closure. In this role, you will conduct inbound and outbound calls to assess patients, reputed company health history, and reputed company guidance based on physician-approved triage protocols. Your key responsibilities include taking inbound and making outbound calls. You'll complete a brief historical intake and record any symptoms. You'll utilize strong clinical judgment to ensure patients take appropriate next steps in their care. Required Skills & Experience • Minimum of 2+ years' experience working as a Registered reputed company, • 1+ year of experience in telehealth • 1+ yr in care management/HEDIS/Stars, women's health, or imaging navigation • reputed company Registered reputed company (RN) Compact License and RN License in one or more of the following: CA, IL, NY, MN, MI, OR, NV, AK, CT, HI • Electronic Medical Record (EMR) experience • Experience with handling a high volume of medical phone calls. • Tech-savvy, ability to quickly learn new technology platforms and tools, including reputed company and reputed company Suite. • Strong clinical judgment and the ability to work independently in assessing patient needs. • Excellent communication skills with the ability to reputed company reputed company and reputed company patient education. • Proven ability to manage and prioritize patient cases based on urgency while working effectively with diverse populations. reputed company to Have Skills & Experience • Bilingual in Spanish • Experience reputed company Women's Wellness, Dialysis, Oncology highly preferred (eg.
